Free ‘Five Wishes’ Workshops
Join Reverend Daybree Thoms for one of the two informative workshops on the ‘Five Wishes’ legal documents. These workshops are open to all community members.
What is ‘Five Wishes’? It’s an essential tool for adults of any age or health status to articulate their preferences for end-of-life care. Also known as an advance directive or living will, ‘Five Wishes’ covers medical, legal, personal, and spiritual considerations, ensuring your wishes are known and respected.
Five Wishes was developed as the first advance care plan (ACP) to address personal, emotional, and spiritual wishes in addition to medical treatment. It’s called the “living will with heart and soul” because the document(s) is based on what is most important—being able to define a roadmap for how you want to be cared for. It has been embraced by families, community groups, faith communities, medical and legal providers, and businesses.
Reverend Daybree Thoms will guide attendees through completing these important legal forms.
